Your Rights as a Patient, Family Member, Substitute Decision-Maker or Caregiver

Learn about your rights as a patient receiving home healthcare services from SJTN Care. Our commitment to dignity, respect, and quality care.

As someone receiving care through Ontario Health atHome, you can expect that all employees, board members, and contracted service providers will uphold and support your rights:

  • 1Respect and Safety: You have the right to be treated with respect and to be protected from all forms of abuse—physical, sexual, emotional, mental, verbal, or financial.
  • 2Dignity and Privacy: Your personal dignity and privacy must be respected, and your independence and involvement in care decisions should be supported.
  • 3Individualized Care: You will be treated as an individual, with sensitivity to your unique needs and preferences, including those related to your culture, language, spirituality, and family background.
  • 4Freedom from Discrimination: You are entitled to receive services without discrimination, in accordance with the Human Rights Code and the Canadian Charter of Rights and Freedoms.
  • 5Culturally Safe Services: If you identify as First Nations, Métis, or Inuk, you have the right to culturally safe care that respects your identity and traditions.
  • 6Clear Communication: You have the right to receive information about your care in a format that is clear and accessible to you.
  • 7Participation in Planning: You have the right to take part in the assessment and reassessment of your needs, and in the development or revision of your care plan.
  • 8Support During Assessments: You may choose someone to be with you during assessments and to help with your care planning and evaluation.
  • 9Coordinated Services: You have the right to support in coordinating your services.
  • 10Consent: You can give or refuse consent for any care service.
  • 11Speak Up Without Fear: You have the right to voice concerns or suggest changes to your care or policies affecting you, without fear of reprisal or discrimination.
  • 12Access to Information: You have the right to be informed about the laws, policies, and procedures affecting your care—including this Bill of Rights—and how to make a complaint if needed.

Your Responsibilities as a Patient

Along with your rights, you also have important responsibilities to ensure safe and effective care:

1Respect for Staff: Treat Ontario Health atHome and service provider staff with courtesy and respect. Discrimination, harassment, or violence—including yelling or threats—will not be tolerated.
2Safe Environment: Help provide a safe workplace by:
  • Keeping the home smoke and vape free
  • Securing pets during visits
  • Clearing snow and lighting walkways
  • Storing weapons or dangerous items safely
3Active Participation: Be involved in developing your care plan.
4Communication: Keep your care coordinator or nurse updated on changes in your health or support situation.
5Availability: Be prepared and available to receive care as scheduled.
6Notice of Absence: Let your care team know in advance if you will be unavailable for a visit.
7Updated Information: Inform us of any changes in your contact details or treatment location.
8Collaborative Problem-Solving: Work with your care coordinator and service providers to address any concerns about your care.
